Yes Prim does die I wept for hours apon end. Here's how it happened.
She is a medic who is sent by the rebels to the Capitol to help the ill citizens. There is a group of Capitol Children outside of President Snow's mannor, and a hovercraft marked with the Capitol logo (that is really from the rebels) sends parachutes down to the children, who think there are treats inside. they grab the parachutes, but half explode and injure some the children. Prim and some medics rush in to help, when the rest of the parachutes explode.
President Coin sends Prim there and then tricks Katniss into thinking that Snow killed Prim.
